Summary:
The Assistant Director of social Services is responsible for the long-term care residents who reside in our facilities.
The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our facility's Social Services Department. As a Assistant Director, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.
Job Functions:
Key Responsibilities:
Assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs.
Participate in discharge planning, resident assessments, and developing social care plans.
Interview residents/families to obtain social history.
Refer residents/families to appropriate community agencies when needed.
Provide consultation to staff and community agencies on meeting residents' social/emotional needs.
Interpret residents' social, psychological, and emotional needs to the care team.
Assist in obtaining resources from community agencies to meet residents' needs.
Ensure confidentiality of residents' protected health information.
Administrative Duties:
Assist in developing and administering department policies and procedures.
Review policies/procedures annually and recommend changes.
Complete required documentation, forms, and reports.
Assist in arranging transportation for residents when necessary.
Provide information on financial assistance programs to residents/families.
Assist in reviewing and updating job descriptions annually.
Delegate authority and make recommendations to the Director.
Standardize methods and provide solutions for residents' social/practical needs.
Education:
The job requires a Bachelor's degree in Social Work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure
