The GUIDE program provides comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their caregivers/support network(s), and enable people with dementia to remain in their homes and communities. This full-time position in an outpatient practice in the Perelman Center for Advanced Medicine will be responsible for supporting older adults with Alzheimer's disease and other dementias, and their caregivers/support network(s). The social worker will train care navigators on common issues affecting patients with dementia and their caregivers, and they will supervise the care navigators, providing ongoing clinical guidance and consultation around working with this population. They will be responsible for ensuring that care navigators are in compliance with all program requirements including regular patient contact, clinical documentation, and resource allotment. They will facilitate multiple monthly caregiver support groups, as well as psychoeducational caregiver classes each year. The person in this position will be part of a team working with neurologists, geriatricians, psychiatrists, APPs and other social workers, including MSW interns, to deliver high-quality, coordinated care. They will work with the program manager to prepare data to be sent to program funders, such as Centers for Medicare and Medicaid. Supervision for clinical licensure (LCSW) is available if requested.Job Description

Job Responsibilities:
-Collaborate and coordinate with other members of the multidisciplinary team internal to Penn and external providers to ensure continuity of care and transitional care management for GUIDE participants
-Train care navigator(s) in providing dementia-competent care and plan additional education as needed
-Supervise care navigator(s) in delivery of case management and supportive services to GUIDE participants (patients and caregivers); provide clinical guidance as needed
-Is responsible for documentation of comprehensive assessment and care plan in patient's medical record, as well as ensuring care navigator(s) is in compliance with program requirements
-Facilitate support groups for patients and family caregivers as well as psychoeducational caregiver classes.
-Carry out professional activities in accordance with the code of ethics established by the National Association of Social Workers.

Qualifications

  • Bachelor of Science, Bachelor of Arts, and 5 to 7 years or Master of Science, Master of Arts, and 3 to 5 years of experience or equivalent combination of education and experience is required.
  • MSW with active social work license (LSW or LCSW)
  • Ability to work independently in a high-volume clinic
  • Past experience working with older adults and/or older adults with dementia
