Title: Health Coach

Location: OH-MASON, 4361 IRWIN SIMPSON RD

*Remote: candidates do not have to be located in Ohio. However, Preferred licensure in Ohio*

Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.

Shift: 9:00am - 6:00pm EST, Monday - Friday (8 hour shift)

The Health Coachis responsible for providing health coaching to members, across all brands, with chronic diseases.

How you will make an impact:
Primary duties may include, but are not limited to:

  • Makes outbound telephone calls to members for follow-up post hospital discharge to conduct behavioral assessments to identify individual member knowledge, skills, and behavioral needs.
  • Coordinates specific health coaching to address objectives and goals as identified during assessment.
  • Implements coaching plan by using behavior change principles to identify member barriers and develop ways to overcome those barriers.
  • Coordinates with the nurse case manager to provide feedback on member goal attainment and clinical issues.
  • Monitors and evaluates the interventions and modifies.

Minimum Requirements:
  • Requires BA/BS in appropriate field of specialization (examples such as Health Education, Exercise Physiology, Respiratory Therapy or Dietician) and minimum of 3 years of related experience in health education, exercise instruction, or patient education; or any combination of education and experience, which would provide an equivalent background.
  • Certification, advanced certification, and/or license appropriate to field of specialty as required.
  • For associates working within Puerto Rico who are member or patient facing either in a clinical setting or in the Best Transportation unit, a current PR health certificate and a current PR Law 300 certificate are required for this position.

Preferred Skills, Capabilities and Experiences:
  • BA/BS in appropriate field of specialization such as Social Work, Nursing and Counseling preferred
  • Experience in health coaching/case management and telephonic services with members with a broad range of chronic disease management preferred
  • Clinical licensure for the state of Ohio - LSW, LISW, LPN, RN, LPC, etc. preferred
  • Strong oral, written, and interpersonal communication skills, problem-solving skills, and computer skills preferred
  • Knowledge of behavioral and/or clinical assessment techniques preferred
  • Health and/or patient education preferred
  • Behavior change techniques preferred
  • Understanding of disease management principles preferred
  • Ohio License preferred
